Case Management

Our Case Management Service is a collaborative effort that involves an assessing of the client’s level of complexity and developing and facilitating a comprehensive plan of care aimed at promoting client safety, stability, and independence and meeting the client’s and family health needs by connecting the clients to services and resources and coordinating and supervising their use of them. Our highly trained and compassionate case managers are committed to providing quality support geared towards recovery and wholeness

Grounded in the philosophy that, when given proper support and education, people can thrive, regardless of mental health conditions, we provide evidence-based curriculum aimed at building knowledge and skills including but not limited to anger management, problem solving, independent living, emotional coping, and management of symptoms of mental illness.

Crisis Intervention

It is our aim to provide a consistent support system for our clients. Therefore, we have a 24-hour hotline available for our clients in case of emergencies and after-hour needs. Life-threatening emergencies should be directed to 911.

Medication Education and Support

In an effort to give our clients everything they need to thrive amid their mental health condition, this service entails taking inventory of clients’ medication as prescribed by their primary physician or psychiatrist and ensuring that the client and family are knowledgeable of medication purpose and side effects and compliant with medication dosage and frequency.

Psycho-social Rehabilitative Services

We believe recovery and rehabilitation is a community effort. Therefore we are committed to walking alongside of our clients as they navigate through difficult circumstances and conditions. This service entails assessing the clients’ needs and support systems in place and connecting the client/family to various services and resources offered in the community including housing, employment assistance and training, coordination of services, etc.

Medication Assisted Treatment (MAT) Program

This program is targeted at individuals who are opioid/alcohol dependent and/or have smoking addictions. Research shows that combining medication and behavioral health services can successfully treat substance use disorders and help sustain recovery. We prescribe FDA-approved medications based on each patient’s individual needs. We want each patient using MAT to make a full recovery and to live an empowered, self-directed life.
